Are you interested in learning more about LHC and our signature program, Leadership Premier? Consider attending a virtual Information Session that will cover details about the 2026 Leadership Premier program. We are also thrilled to welcome Dr. Matt Castner, Director of Population Health with the Johns Hopkins Howard County Medical Center.
Howard County Medical Center (HCMC) realizes that healthcare is a team sport. In the hospital, that team is comprised of doctors, nurses, and other support staff. In the community, though, that team consists of physicians and a range of other supportive organizations helping people stay healthy. Dr. Castner will share more information about how HCMC and its Community Care Team work with recently discharged patients to help connect them with community-based services and address Social Determinants of Health or the conditions people live, worship, and play in.
